Mosaic Life Care turned price transparency into a revenue channel by letting patients purchase care at clear, upfront prices before they arrive. Across three hospital campuses in Northwest Missouri, the regional health system has seen strong, sustained voucher revenue growth and engaged more than 2,000 new patients through Tendo Marketplace.
Why cost uncertainty keeps patients from care
Cost uncertainty is one of the most common reasons patients delay care. Nationally, 28% of adults say they skipped or postponed needed care in the past year because of cost, according to KFF, and 33% of covered workers were in a high-deductible plan with a savings option in 2025, per the KFF 2025 Employer Health Benefits Survey.
This health system saw the problem firsthand. It serves a largely rural region with a significant uninsured and underinsured population, and patients had no upfront prices for imaging, lab work or specialist visits. Many on high-deductible plans deferred routine care, and post-service billing disputes added collections risk. Any fix also had to work inside the system’s existing EMR.
What is a patient-facing point-of-sale platform?
A patient-facing point-of-sale platform lets patients see a price, pay for a service and hold proof of payment before their appointment. In Tendo Marketplace, that proof is a care voucher: a prepaid purchase of a defined service at a transparent, pre-negotiated price. The health system collects payment upfront, and the patient arrives knowing what they owe. Learn how Tendo Marketplace works.
How the health system deployed Tendo Marketplace inside its EMR
The health system deployed Tendo Marketplace across all three hospital campuses and dozens of provider locations, covering more than 10 service lines, from imaging and lab work to OBGYN, cardiac imaging, orthopedics and bariatrics. Voucher status appears directly in the existing EMR patient record, and front desk teams handle redemption without added IT overhead.
“The Epic integration was the piece that made this scalable. Our staff didn’t have to learn a new system — Tendo fit right into the workflow they already knew. That’s what allowed us to go live across all three campuses and drive real adoption.”
—Drew Keesbury, Chief Operating Officer, Mosaic Life Care
Results: revenue captured earlier in the care journey
The health system reported strong, sustained voucher revenue growth to date. Imaging and radiology led all service lines, followed closely by OBGYN.
- More than 2,000 new patients engaged through Tendo Marketplace.
- Less AR exposure. Upfront voucher payment reduced write-offs.
- Consistency at scale. Patients and staff had the same experience across all three campuses from day one.

What health systems can take from this rollout
This rollout offers a replicable model for health systems that want a patient-facing marketplace without disrupting clinical workflows. A live EMR integration, a lean revenue cycle team and executive sponsorship carried it across three campuses. For rural and regional systems, upfront pricing addresses affordability and access at once. See how rural health systems can improve access and affordability.
“Patients shouldn’t have to choose between accessing care and affording it. We needed a solution that met people where they are — transparent pricing, flexible payment, and zero friction at the point of need.”
—Drew Keesbury, Chief Operating Officer, Mosaic Life Care
